Consent Form:
1. Mother
2. Father
3. Child (If child is small, then consent form will be filled by Mother)

Documents

  1. Aadhar Card of Parents
  2. Birth Certifcate of Child
  3. 4 Passport size photograph of each
  4. 2 consent form will be filled by each applicant (Father, Mother & Child)

Note: This test shows the Biological relationship on Father & Child only.

Test can be done, if child is born, Prenatal testing is not done
